Job Description
POSITION:  CASE MANAGER AT PORTAGE PLACE GROUP HOME
 
SALARY: This is a full time salaried exempt position.
 
SUPERVISION RECEIVED: Works under the general supervision of the Group Homes Coordinator.
 
SUPERVISION EXERCISED: Responsible for day to day supervision of youth related services.
 
GENERAL DUTIES:  Is responsible for youth services insuring compliance with the home’s policies, procedures, treatment plans, interventions, group sessions, and Activities of Daily Living Skills.      
 
EXAMPLES OF DUTIES: (These are typical duties and are not intended to be all inclusive or restrictive in nature)
 * Liaison with community organizations, referral / placement agencies and school system.
 * May work in a direct care worker capacity as necessary. 
*  Provides assistance or consultation to staff on an around the clock basis in regards to youth in home.
*  Resolve youth related milieu related disputes. 
*  Ensures that case records/ youth files contain required documentation.
 * Will conduct social skill and other groups. 
*  Will conduct youth interventions.
 * Organize youth activities. 
*  Supervise and help at tutor time.
 * Reports to Group Home Coordinator weekly schedule and for vacation / sick time away from work. 
 
MINIMUM QUALIFICATIONS:
 
EXPERIENCE:  Prior experience working with troubled, severely emotionally disturbed and / or delinquent youth, preferably in a residential “type” group care setting. Prior supervision experience helpful.  
           
KNOWLEDGE:  Must have working knowledge of adolescent behavior and needs, knowledge of crisis management, conflict resolution, and counseling techniques.  
 
ABILITIES:  Ability to develop and implement creative behavior intervention strategies, to communicate and relate with youth of divergent backgrounds, staff , social service agencies, and the community at large. The ability to organize one’s time efficiently and wisely, and to communicate effectively orally and in writing. The ability to work under stressful conditions.
 
SKILLS:  Demonstrate problem solving skills
 
EDUCATION:  A Bachelor’s degree in Social Work / the behavioral science filed, or a combination of experience is equivalent to a Bachelor’s Degree.  Each year of post-secondary education in human services equals one-year of experience.  
 
OTHER:  Must have a valid Montana State Driver’s License, complete the Personal Statement of Health Form for Licensure, and pass background checks required by the State of Montana.  Any adverse reports / findings will disqualify you for employment.
 
PHYSICAL DEMANDS: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.  Reasonable accommodations may be made to enable individuals with disabilities to perform the essential job functions.  While performing the duties of this job the employee is frequent5ly required to sit, talk or hear, stand; walk; and use hand to handle or operate objects. The employee is occasionally required to climb or balance, stoop, kneel, crouch, lift, reach with hands or arms and run.  The employee must occasionally lift and /or move up to 40 lbs.  The employee must be capable of working a varied shift schedule. Specific vision abilities required by this job include close vision, distance vision, and the ability to adjust focus.
